Channel 1's Mail-To Form With Infinite Fields

This page contains information specific to the Channel 1 system, for use by registered Channel 1 users. We now have a script that allows you to create a customized mail-to form for your Web page. In addition to just plain text fields (as many as you want!) you can also add check boxes, radio buttons, and pop down menus. Visitors to your Web page fill out the form and all the values are sent to you as an e-mail. OK, here's how to do it.


1. To start with, Click here to see what our Mail-To Form looks like. Here is the HTML code for our Mail-To Form.

2. Copy the HTML code into any text editor such as NotePad, Word, or Simpletext. Using your text editor, change the recip field from www@user1.channel1.com to your email address, for example tester@user1.channel1.com. Now you can modify the form as you like. Remember, there is no limit to the number of text fields, check boxes, or radio buttons that you may put on your form. NOTE: There are some required fields:

• The e-mail address of the sender must be named "from". For example,
  
  Name:<input name="from" size=45>
  
  If, for some reason, you don't want the user to enter an e-mail address, you can make the field hidden. For example,
  
  Name:<input type = "hidden" name="from"...>
  
  
• You must designate a follow-up page. This page must either be a full URL(beginning with http://, ftp://, .etc) or a partial URL containing only the relative pathname part of it (starting with a /) The latter method is usually much more efficient. For example,
  
  <input type="hidden" name="followup-page" value="http://www...">
  
  
• You must have a text field(either TEXTAREA or INPUT TYPE) named "body". If you do not want one of these to appear on your page the field can be hidden. If you chose to do this you must define a VALUE. For example,
  
  <TEXTAREA name = "body" ROWS=10 COLS=45>
  or
  <input type="hidden" name = "body" VALUE="...">
  
  

That's it! Now, when the form is used, you will receive an e-mail message with the values of items selected. For example,
